Accommodation Options for Adults with a Learning Disability/additional needs and or Autism in Thurrock

Transitional/Move on support

Description: Supporting adults with a Learning Disability/additional needs and or Autism to experience a supported living environment with a view to a potential move to their own supported accommodation.

 

1. The LD project

Provider name: Thurrock Council/Thurrock Lifestyle Solution CIC

The service is provided at five locations across Thurrock. 

The accommodation comprises of five three bedroom houses which are located in Council-owned sheltered accommodation schemes across Thurrock.

Each property will accommodate two people and (initially), a sleep-in carer at night. Thurrock Lifestyle Solutions (TLS) provide the support and Thurrock Council is the landlord.

The aim of the service is to promote independent living for people who:

a) Have been living in residential care but who have been identified as having needs that could be met by the provision of lower levels of support

or

b) Have the need to experience living in a supported housing environment in order for them to develop life skills that will enable them to sustain independent living in the local community e.g. people in transition or living with parents.

  • Assessment of eligibility for the scheme

The scheme is available to applicants over 18 years of age.

The nature of the scheme is designed to prepare for independent living in the community.  It is anticipated that all residents will work with TLS in readiness for them moving on into independent living. 

Therefore:

  • Applicants will need to be assessed as having the capacity to work with support workers to achieve the goal of moving on to live independently into the community.  Assessments will be carried out by Adult Social Care (ASC) and by TLS to determine suitability for the scheme.  As far as possible, ASC will try to “match” service users in a property, to ensure compatibility.
  • Applicants must be willing to engage and work with support workers to achieve the skills necessary to sustain independent living.
  • Reviews of service user progress in the scheme will be carried out regularly both by ASC and TLS staff.  Reviews of performance and quality of the scheme will be carried out by the Council’s Contracts Team and/or Commissioning Team. 
  • When the service user is deemed ready to move on, then if social housing is the preferred route for re-housing, a report would be sent to the Housing Needs Manager for consideration for the awarding of a Silver Banding.  This will give priority to the individual to move-on from the scheme.
  • Should reviews indicate that the individuals are not progressing in the scheme, then an alternative placement will be made available by ASC.

 

2. Transition House

Thurrock Lifestyle Solutions CIC has two transition houses located in Hathaway Road, Grays.

The ethos is to train people to live independently. During the time in the transition House, Thurrock Lifestyle Solutions can also help people with:

  • Employment Support
  • Provision of PA s
  • Training
  • Brokerage
  • At Thurrock Lifestyle Solutions CIC we pride ourselves on treating everyone as an individual. This means that one size does not fit all and we like to spend time supporting people to find the right level of support that keeps your loved one safe and happy living in the community. Independent Living does not mean living a lonely life on your own with no support.

We have access to a number of properties, developers, housing associations and individual landlords that can help you find the right home in the right place. We run for the Council a ‘Move On’ project that gives people a chance to safely try living independently with lots of support.

As part of this we offer ASDAN training in independent living skills, from cooking through to paying bills, but ultimately if someone needs 24/7 support we can help you find the right staff to support them in the safe way you would want.

For us, it is all about supporting people in the 5 ways to well-being  - thus enabling people to;  Connect, Be Active, Keep Learning, Give to Others, and Be Mindful. We believe these will keep your loved one content and leave you feeling that we are a partnership in supporting them to live the lifestyle of their choice.

 

3.Sunnyside House

Provider name: Sunnyside House Ltd

This is a specialist young people residential home for adults with learning disabilities and mental health needs that focuses on independent living skills in order for people to move on to lower supported housing across all areas of Thurrock.

This provider supports resident learners to be as independent as possible by teaching life skills through “My Life Programme”, the programme covers all aspects of life from managing money to dealing with conflicts.

This is done by using a tailored way of teaching that suits each person individually, with some people gaining more from a classroom environment and others preferring hands-on learning.

Residents move from the residential unit when ready to the Sunnyside flats where they experience independent living within arm’s reach of the home. A number of innovative assistive technology solutions have been fitted into these flats.

Over the last few years a number of people have moved on from the independence flat and Sunnyside House continues to support them in their own accommodation.

Shared lives

Description: Shared Lives carers can support you or your loved one within a normal family environment. You will be able to do the things you currently do and see the people you like to see and enjoy spending time with.

You will be able to live with someone who has similar outlook or interest. You could live with a Shared Lives Carer long term or come for regular short breaks.

There are currently a number of shared lives arrangements within Thurrock. We have shared lives carers who have been recruited and are waiting for someone who is a good match who wants to stay in their home.

The matching and arrangement process is very important to ensure that the appropriate home and shared lives carer is identified for you. All shared lives carers are carefully vetted and trained. The scheme is currently rated outstanding by the Care Quality Commission.

Provider name: Better together

 

Supported Living

Description: Supporting adults with a Learning Disability/additional needs and or Autism to live independently in their own home, with appropriate support to help them manage their own tenancy and achieve greater freedom and control in their lives. Support can be provided in a person’s own house, their flat or in a shared living environment.

Services can range from occasional visits from a support worker, flexible support during the day or in the evening, overnight and/or at weekends, right through to 24 hours a day. Some services might be shared if more than one person with support needs live together, for instance a couple sharing a flat.

 

 

1. Chichester Road

Provider name: Thurrock Council Housing department/individually commissioned support providers.

This is a property in Aveley with 8 individual flats mostly for people who have moved from the family home or supported living accommodation.  

The 8 flats, in two low rise blocks, have undergone a complete transformation to make them suitable for people with learning disabilities moving from residential care or moving through the transition house and move on projects. 

The flats are fitted with up to date energy efficient heating and electrical systems, including cabling to support the use of Assistive Technology. 

The 4 ground floor flats are adapted for use by people who use wheelchairs or have impaired mobility.

This accommodation gives people with learning disabilities and autism the possibility of living independently in modernized, technology enabled homes and was made possible by the award of a grant from the Housing & Technology for People with Learning Disabilities Local Authority Capital Fund.

Residents can use their DP monies or have council commissioned service depending on the outcomes of their assessed needs.

 

2. Medina Road (currently in development, likely to be ready for occupation in the summer 2021)

Provider name: Peabody Housing Association/individually commissioned support providers.

Medina Road is an autism specific housing scheme that comprises of six new wheelchair standard homes for people with autism being developed by Peabody Housing Association with the support of Thurrock Council.

No two people with autism are the same, every individual needs their own assessment, care plan and bespoke support.

The care and support service will consist of staff available in the scheme 24 hours a day, as well as personalised care and support services, unique to each individual tenant, purchased by the tenant using their personal social care budgets.

This ensures that tenants are able to exercise choice and control over their daily life and routine, and put into place a set of services from teams or individuals who have specific skills or abilities that help tenants achieve their own goals and aspirations.

 

3.  Community based Supported Living

Provider name: Thurrock Lifestyle Solution CIC

Thurrock Lifestyle Solutions supports a significant amount of adults with a Learning Disability/additional needs and or Autism to live independently in their own homes, across Thurrock.

 

4. Thurrock Accommodation Services

Provider name: Peabody Housing Association

Thurrock Accommodation Services provides high quality housing related support for adults with disabilities, including physical, sensory and people with learning disabilities covering 22 placements.

The accommodation is set within three schemes located across the Thurrock area and customers are issued with Assured Short hold tenancy agreements (AST).

  • South Ockendon: 10 placements (Six single self-contained flats and two doubles)
  • 2 x Stanford – Le Hope: 12 placements

The main principles of supported living are that people with varying support needs have greater control and autonomy over the type of support they receive and how they live their lives. 

We provide support across a 7 day period including the provision of sleeping in support in two of schemes.

For customers assessed with complex/ long term support needs this can be delivered on an ongoing basis through the provision of direct payments hours.

This aspect of the customer support is invoiced separately and can be provided by Peabody in conjunction with external agencies offering the customer greater choice and control over their support.

 

  • Chafford Hundred – 10 placements (Six single self-contained flats and two doubles)
  • Grays – 8 self-contained flats, Landlord is Estuary Housing. Peabody are the support provider Client group- people with mental health 18+ providing low level housing related support.

 

  

4. Care Management Group – Supported Living

Provider Name: Care Management Group

The care management group supports a small number of adults with a Learning Disability/additional needs and or Autism to live independently within Thurrock.

 

5.Satash Com Care Projects  

Provider Name: Satash Com Care Ltd

Satash Com Care supports a small number of adults with a Learning Disability/additional needs and or Autism to live independently within Thurrock.

 

 

Residential Support

Description: Residential homes for adults with a Learning Disability/additional needs and or Autism provide 24 hour a day, 7 day per week holistic support and care.

Ensuring all aspects of a person’s life are supported, providing them with a safe and secure environment in which all their needs can be met, including personal care, health care, emotional and social well-being.

Residential care model is legally defined in the care standards Act 2000 as an establishment that provides accommodation and personal care which is defined as assistance with bodily functions such as supporting people to eat, bathing and toileting when required.

They also endeavour to support people to be included in their local community and make use of leisure and recreational facilities as active citizens. Whilst supporting them to both maintain and develop relationships with family and friends.

There are 19 residential homes for working age adults (18 to 65+) situated across all geographical areas of Thurrock.

17 of these homes are primarily for adults with a Learning Disability/additional needs, some will also support people with Autism and individuals with Mental Health issues/dual diagnosis.

2 are primarily for people living with Mental Health issues.

RESPITE SERVICES

There are few care homes in Thurrock that would consider taking people in for respite if they have a vacancy.

Description: Providing short term respite care for people throughout the year that can be booked as individual days or blocks of time up to four weeks accommodation and support for people with learning disabilities. In order to provide respite for family carers.

This service in Aveley provides accommodation and support for up to four people with learning disabilities. Ensuring all aspects of a person’s life are supported, providing them with a safe and secure environment in which all their needs can be met, including personal care, health care, emotional and social well-being.
