SEN - Preparing for Adulthood (SEN PFA) Team - Education

The main responsibility and focus of the team

  • attend and provide support at EHCP Annual Review meetings, updating and amending EHCP’s as required
  • provide information, advice, and guidance ahead of and during the educational transition

Roles and responsibilities within the team include

  • Providing advice and guidance in the key areas of Preparing for Adulthood
  • case management – writing and amending EHCPs and attending annual review meetings for 16-25 year olds (year 12 upwards), supporting phase transitions and the ‘mover-in or out’ processes
  • liaison with other professionals who are supporting young people including Social Care and Health, this includes attendance at a variety of statutory meetings
  • providing CEIAG (Careers, Education Information Advice and Guidance) to SEND pupils in year 9 and 11 and post 16 this includes a careers interview and action plan.
